Veterans Health Checks

Veterans Health Checks, or Post-ADF Discharge Health Assessments, are the cornerstone of establishing your overall health care, establishing your ongoing health requirements and beginning to inform an ongoing DVA claims. 

This consult is usually bulk billed, is fully funded by Medicare or DVA, and allows us the time required to set your health care up for success. 

Dr Perkins is an ex -ADF MO and Senior Clinician, with 20 years Military experience, who understands the nuances and complexity of ADF Veteran Healthcare, and speaks your language. 

  • Senior Clinician – Contracted Health Professional  with HCA and BUPA at RAAF Amberley and Enoggera Army Hospital, 2013-2020.
  • Former DVA Contracted Medical Advisor, 2013-2014
  • 16 year experience in the Australian Defence Force, 1996 – 2017

Veteran Health Checks

We support health checks for veterans to help them stay well after they move into civilian life. 

We offer the:

  • One-off Veteran Health Check for a veteran to have once, any time after they leave the Australian Defence Force (ADF)
  • Annual Veteran Health Check for a veteran to have each year for up to 5 years after they leave the ADF (if after 2019)

Your appointment will be conducted over 2-3 sessions, depending on the copmlexity of your case.

A list of your medicines

An accurate summary of your medicines, accurate at the time of the consult, will help in organising the clinical care of your case. 


Discharge Documentation

A list of your discharge documentation – PM552 and all related documentation will be helpful if brought to the consult.

ADF Post-Discharge Health Assessment

The Australian Defence Force (ADF) Post-discharge GP Health Assessment was developed by the Department of Veterans’ Affairs (DVA) as part of the Veteran mental health strategy: A ten year framework 2013–2023 initiative.

The health assessment was designed to assist former ADF personnel to access primary healthcare after their discharge.

It consists of the following components:
